L’accident de Tchernobyl

Deux ordres de conséquences sanitaires de l’accident de Tchernobyl sont distingués. Les soins aux personnes irradiées ont posé deux problèmes: celui de l’évaluation des doses reçues et celui du traitement, en particulier les greffes de moelle. L’étude des conséquences à long terme nécessite l'évaluation du risque de cancer induit. Ce risque est discuté pour les différents groupes concernés : la population proche de Tchernobyl, la Russie d’Europe et l’Europe ; il paraît très faible pour les deux derniers groupes. Il justifie des études prospectives dans le premier groupe, bien qu’étant donné les doses relativement faibles, il est peu probable que l’on parvienne à mettre en évidence un excès de cancer.
Abstract
Two types of health effects of the Chernobyl accident are discussed. The management of the individuals who have received high doses of radiation raises two problems : 1) the biological and clinical dosimetry, and 2) the treatment, in particular the role of bone marrow allografts. The assessment of the possible longterm effects relies on the evaluation of the carcinogenic effect of low radiation doses. Three groups should be considered : the population present at the time of the accident in the immediate vicinity of Chernobyl, the population in the European part of USSR, and the European population. The dose was so small in the two latter groups that a risk assessment is extremely debatable. The effect on the population around Chernobyl should be carefully and prospectively evaluated despite the fact that, probably due to the small dose received, any effect would be extremely difficult to detect.
